Lifecare Home Health - - Responsibilities: Provide direct patient care to patient under the direction of the RN and according to the Aide Plan of Care.; Assist with safe transfers and ambulation per Hospice policy.; Report changes and document pertinent information and care rendered to patient.; Maintain infection control principles and provide a clean, safe, and comfortable environment.; Perform on-call/after-hours responsibilities and provide on-call services as assigned.
